• All current medications, in their original bottles, or a detailed list.
• Insurance card(s) and Photo ID. 

All Annual Physical appointments for established or new patients cannot be made online. Please contact our office for scheduling.


Please be advised that our practice is not able to accept New Patients with any form of TennCare insurance at this time.

Also, please be aware that any patient seeking pain management will be referred to a specialty office, as we do not practice pain management in-house.

To reschedule an appointment, call us directly 615-792-1911.
